These Terms of Service govern use of the Elevate Health & Wellness LV website, appointment-request forms, and related communications. By using this website, you agree to these Terms and our Privacy Policy.
1. Website information is not medical advice
Website content is general educational information. It does not replace evaluation, diagnosis, or treatment by a qualified healthcare professional and does not create a provider-patient relationship. A provider-patient relationship begins only after required intake, clinical evaluation, consent, and acceptance by an authorized provider.
2. Emergencies and sensitive information
Do not use this website, its forms, email, or text messaging for emergencies. Call 911 or seek immediate medical care for an emergency. Do not submit detailed symptoms, diagnoses, medical records, insurance information, payment-card data, or other sensitive medical information through general website forms.
3. Eligibility and clinical decisions
Submitting a request does not guarantee an appointment, prescription, medication, treatment, result, or service. Eligibility depends on an individualized clinical evaluation, applicable law, provider licensure, service location, medical history, and professional judgment. A licensed provider may decline, modify, or discontinue care when clinically appropriate.
4. Telehealth, prescriptions, and delivery
Telehealth and prescribing are available only where permitted and only after applicable identity, location, consent, and clinical requirements are satisfied. Prescription products require a valid prescription and are dispensed by an authorized pharmacy. Product and delivery availability varies by patient location, clinical eligibility, pharmacy capacity, and law. Shipping estimates are not guarantees.
5. Payments, cancellations, and refunds
Prices, payment terms, recurring charges, cancellation rules, and refund eligibility must be presented before purchase or enrollment. Service-specific written terms provided at booking or checkout control if they conflict with general website information. Contact us before paying if a charge, renewal, cancellation, or refund term is unclear.
6. Text messages and email
If you separately opt in to marketing text messages, message frequency varies and message and data rates may apply. Reply STOP to opt out and HELP for help. Consent to marketing messages is not a condition of purchase. Operational messages related to a request or appointment may be sent as permitted by law and your preferences. Carriers are not liable for delayed or undelivered messages.

You may not misuse the website, interfere with its operation, attempt unauthorized access, submit false information, impersonate another person, or use website content unlawfully. We may restrict access when reasonably necessary to protect patients, staff, systems, or legal obligations.
8. Third-party services
The website and care-coordination process may use third-party scheduling, communications, analytics, payment, telehealth, laboratory, pharmacy, or delivery services. Elevate currently may use LocumTele, LLC and Doctors Now, LLC for telehealth technology, clinical infrastructure, provider coordination, provider staffing, medical oversight, or related support, depending on the service. The entity and licensed clinician responsible for clinical care will be identified during intake and consent. Independent providers remain responsible for their clinical judgment and services. Third-party terms and privacy practices may apply.
9. Intellectual property
Unless otherwise stated, website text, branding, graphics, and design are owned by or licensed to Elevate Health & Wellness LV. You may view them for personal, noncommercial use but may not reproduce or exploit them without permission.
10. Disclaimers and limitation of liability
The website is provided on an “as available” basis. To the extent permitted by law, Elevate Health & Wellness LV disclaims warranties not expressly stated and is not liable for indirect, incidental, or consequential losses arising solely from website use. Nothing in these Terms limits rights or liability that cannot legally be limited.
11. Changes and governing law
We may update these Terms by posting a revised effective date. Continued website use after an update constitutes acceptance of the revised Terms. Unless controlling law requires otherwise, Nevada law governs these Terms.
